Our Aims

Strive Health is designed with a purpose — to overhaul a flawed kidney care system. We are dramatically transforming the lives of kidney disease patients through early detection, engagement, and all-inclusive coordinated care. The Strive model is powered by a high-touch care team that collaborates with local providers and extends the entire care pathway from chronic kidney disease through end-stage kidney disease, using comparative and predictive data and analytics to identify patients at risk. Strive Health's interventions considerably decrease the rate of unexpected dialysis crash, reduce inpatient use, and significantly enhance patient outcomes and experience. Join us on our journey as we become THE point of attraction for top talent in the healthcare community and set a new standard for kidney care.
